Join fellow Member Kim Delp, Senior Director of Home & Community Based Services for the Northern Multi Service Agency of Allegheny County, PA, for this Leadership Academy webinar on Thursday, January 26. Kim will teach you how to develop data collection strategies that could enable your program to compete for healthcare contracts in your local area. Kim will share how her program innovated with the development of a mobile application designed to capture participants’ “change of condition,” demonstrating how regular monitoring prevented unplanned hospitalizations, met consumers’ needs and kept clients in their own homes. Kim will share findings from an effective post-acute discharge nutritional support/change of condition pilot project that was able to quantify the cost savings. You’ll even walk away with a model evaluation strategy that you can use to examine your own program and identify future data collection requirements.
