Creating Age-Friendly Health Systems: An Invite to Join the Action Community

The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the AHA and the Catholic Health Association of the United States, developed the 4Ms Framework for Age-Friendly Care, an evidence-based model to support the needs of the aging community in a clinical setting.
